Hospice Nurse

• Responsibilities:
  • Provide direct nursing care to patients in their homes or facilities.
  • Develop, implement, and evaluate patient care plans.
  • Conduct health assessments and monitor patient progress.
  • Collaborate with interdisciplinary teams to ensure holistic care.
• Qualifications:
  • Registered Nurse (RN) with a valid license.
  • Experience in hospice or palliative care preferred.
  • Excellent communication and organization skills.

Social Worker

• Responsibilities:
  • Provide counseling and support to patients and families facing end-of-life issues.
  • Assist in accessing community resources and financial assistance.
  • Develop care plans that address psychosocial needs.
  • Facilitate family meetings and support groups.
• Qualifications:
  • Master’s Degree in Social Work (MSW) and licensure.
  • Experience in healthcare settings; hospice experience is preferred.
  • Strong interpersonal skills and empathy.

Certified Nursing Assistant (CNA)

• Responsibilities:
  • Assist patients with daily living activities such as bathing, dressing, and meal preparation.
  • Monitor and report changes in patient conditions to the nursing staff.
  • Provide companionship and emotional support to patients and families.
• Qualifications:
  • Certified Nursing Assistant credentials.
  • Compassionate attitude and dedication to patient care.
